Tesamorelin is a peptide: a string of amino acids joined together, in this case 44 of them. It copies growth hormone-releasing hormone, often shortened to GHRH. The brain makes GHRH in a region called the hypothalamus and sends it a short distance to the pituitary gland.
A Natural Messenger, Made Tougher
Natural GHRH works for only a few minutes before enzymes in the blood cut it apart. Tesamorelin keeps the same chain of amino acids but carries a small fatty-acid group, called a trans-3-hexenoyl group, at its front end. That cap shields it from those enzymes, so one injection keeps working for longer.
The Chain of Events
- Step one: tesamorelin binds to GHRH receptors on cells in the pituitary gland.
- Step two: the pituitary releases more of its own growth hormone, in pulses, much as it would naturally.
- Step three: the liver answers by making more IGF-1, a growth factor that carries many of growth hormone's effects.
- Step four: growth hormone helps the body burn stored fat, and visceral fat seems especially sensitive to it.
Why Not Just Give Growth Hormone?
Growth hormone itself does reduce belly fat, but injecting it directly can raise blood sugar and cause swelling and joint pain. Working one level higher, at the pituitary, leaves the body's own feedback brakes in place. The hope was a gentler effect, and in the main trials blood sugar did not change in a meaningful way on average.
The catch is that the effect lasts only as long as treatment does. When people stopped, the visceral fat came back within months.
The Main Points
- Tesamorelin is a 44-amino-acid copy of the hormone GHRH, made to resist breakdown.
- It makes the pituitary release more growth hormone, which raises IGF-1.
- Its approved purpose is to reduce deep belly fat in adults with HIV.
- The benefit fades once it is stopped.
